SilverChain is seeking a Care Partner to support older Australians in their homes in the Western Adelaide & Surrounds region. You will conduct client assessments, develop personalised Care Plans and coordinate services to meet government-funded aged care requirements.
You will build trusted relationships with clients, families and care teams, advocate for client needs, and help clients stay safe, independent and connected at home through cooperative, multi-disciplinary work.
