Community Accessability is a not-for-profit organisation dedicated to supporting the independence and inclusion of people with disabilities and the elderly. With six offices across regional Victoria, we provide transport, community access, and in-home services through the NDIS, HACCPYP and Aged Care programs.

Are you an experienced home care skilled who builds strong relationships and takes pride in helping people access the right support?
Community Accessability is seeking a compassionate and organised Care Partner to join our Aged Care Services team. You will work alongside older people and their families to coordinate personalised Support at Home services that reflect their needs, goals and choices.
About the role
As a Care Partner,
you will be the primary contact for an allocated client caseload, guiding clients through their care journey and ensuring their services remain responsive as their needs change.
Working closely with our Aged Care Services Manager, RN Clinical Governance Lead, Team Leaders and associated providers, you will:
Develop, coordinate and review individual care plans with clients and their chosen supporters.
Prepare and monitor client budgets, identifying underspending or overspending and working with clients to make informed decisions.
Maintain regular contact with clients and respond to changes in their needs, preferences and circumstances.
Coordinate services and referrals, liaising with support workers, allied health professionals and associated providers.
Assist scheduling staff to address service needs and maintain continuity of support.
Respond to prospective client enquiries about services, pricing and commencement.
Recognise and promptly escape clinical concerns, incidents and safeguarding issues.
Maintain accurate and timely client records in our Customer Management System.
Contribute to quality improvement and support recruitment and onboarding activities when required.
About you
You bring warmth, sound judgement and the ability to turn a client's goals into a practical plan.
You communicate clearly, follow through on commitments and work well with others.
We are looking for:
At least two years' experience in home care case management or care coordination.
Experience developing care plans and managing individual client budgets.
An understanding of Support at Home, aged care reforms, Quality Standards and client rights.
Strong organisational skills and the ability to manage competing priorities.
Confidence working with clients, families, carers and service providers from diverse backgrounds.
Good written communication and computer skills, including experience with client management systems.
A current driver licence and willingness to travel for client visits.
A relevant qualification in aged care, community services, case management or a related discipline is desirable.
