The Queensland Government's Cairns and Hinterland Hospital and Health Service is seeking a skilled physiotherapist to join the Transition Care Program. You will provide comprehensive assessments, develop intervention plans and coordinate case management for referred clients.
You will liaise with clients, families and other health professionals, deliver evidence-based programs for frail elderly clients and contribute to clinical practice improvements and protocols.
