Resolute Support is seeking a dedicated Case Manager to coordinate participant supports in our Community Based Program. You will build robust relationships with participants, their families, and Support Workers to ensure care plans meet needs and funding is optimised.
With a focus on high-quality service delivery, you will maintain accurate records in line with NDIS requirements, participate in team meetings and contribute to continuous improvement across the service.
