Status:
Full Time - Monday-Friday: 9am-5pm

THE ORGANISATION
Yerrabi Yurwang Child & Family Aboriginal Corporation (est. ****) and its affiliates Yerrabi Yurwang Health Hub and Yerrabi Yurwang Housing (est. ****) (all referred to as Yerrabi Yurwang) were established due to the high unmet health and wellbeing needs of the local Aboriginal Community residing in Canberra's northwestern Ngunnawal region. Yerrabi Yurwang means to 'walk strong' and was bestowed upon the organisation by our esteemed and respected Ngunnawal Elder Aunty Agnes Shea OAM, who was also a Founding Board Member and our Inaugural Patron. Yerrabi Yurwang operates in the Northwest of the Australian Capital Territory, part of the traditional lands of the Ngunnawal People. Yerrabi Yurwang is an Aboriginal Community Controlled Organisation (ACCO), which means it is owned and operated by Aboriginal People, for Aboriginal People, and is committed to the principle of self-determination whereby we engage and empower Aboriginal People and their Families who utilise Yerrabi Yurwang's services and programs.
This role also requires the successful applicant to obtain a Community Agency Employee Consent (CAEC) assessment for clearance as per ACT Childcare Youth & Families - Health & Community Services Directorate, guidelines and requirements. CAEC requirements are – completion of application and provision of current WWVP; current National Police Check; current Driver's License; current Drivers history and 10 years residential address history.
THE ROLE
Yerrabi Yurwang Child is recruiting for a Family Practitioner, Domestic Family & Sexual Violence (DFSV) to provide case management to Woman and Children who are or at risk of homeless due to experiencing DFSV in collaboration with Yerrabi Yurwang programs, key partners and other services within the community. This program will also provide trauma-informed, family-centred case management and outreach support to women and children residing in crisis and transitional accommodation homes. The role focuses on safety, recovery,



and empowerment through integrated service delivery, cultural safety, and strong partnerships with specialist agencies. In partnership with DFSV and Cultural specialists the role will offer wholistic case management services to woman, children and significant loved ones to address their housing insecurity due to DFSV through crisis and transitional housing supports, trauma-informed and family focused case management planning. As Family Practitioner you will hold a caseload of approximately ***** families, with consideration given to the complexity and acuity of needs. support.
THE PURPOSE
The purpose of the role is working and collaborating through appropriate partnerships to undertake comprehensive risk assessments and safety planning to reduce harm and prevent future violence, while delivering trauma-informed, culturally aware practice that supports clients' connection to family, community and culture. It works with individuals and families experiencing complex needs, including engagement across legal, housing, and child protection systems, and maintains strong working knowledge of the ACT Domestic and Family Violence service system and referral pathways to ensure coordinated, effective support.
KEY RESPONSIBILITIES
Case Management & Trauma-Informed Support
Deliver wholistic, strengths-based case management tailored to the needs of women and children escaping DFSV.
Develop and implement individualised care and safety plans in collaboration with clients and partner agencies.
Provide in-home and community-based outreach support, including emotional support, advocacy, and practical assistance.
Integrated Service Coordination
Undertake comprehensive risk assessments and safety planning to reduce harm and prevent future violence.
Work collaboratively with Yerrabi Yurwang internal programs.
Work collaboratively with housing providers, health services, legal support, child protection systems, and other relevant agencies.
Facilitate warm referrals and case conferencing to ensure wraparound support.
Maintain knowledge of the ACT Domestic and Family Violence service system and referral pathways
Maintain strong partnerships with specialist cultural and safety planning services.
Apply culturally informed, trauma-responsive practice that supports connection to family, culture and community.
Embed trauma-informed, culturally protected, and family-centred approaches in all aspects of service delivery.
Monitoring & Reporting
Maintain accurate case notes, risk assessments, and data in line with organisational and funding requirements.




Participate in reflective practice, supervision, and continuous improvement processes.
Capacity Building & Leaming
Engage in ongoing professional development, including access to learning and development opportunities provided by the organisation.
Contribute to team learning and service development through knowledge sharing and collaboration
Staff Development
Attend Team Meetings.
Undertake professional development as per your Workplan and Yerrabi Yurwang requirements, including program specific core training.
Administration
Keep up to date. factual and relevant case notes as required.
Ensure case notes/files meet organisational and legislative requirements.
Quality Assurance
Adhere to Work Health and Safety policies and legislation.
Adhere to agency policy and procedures and work within relevant legislative requirements.
Contribute to a positive workplace environment.
Participate in planning days and team meetings.
Key Performance Indicators (KPIs)
Contribute towards contracted KPls as per funding/grant requirements.
Contribute to program data and reporting.
Deliver a comprehensive service to eligible individuals.
Adhere to the program model and sector guidelines.
Monitor and review individual performance and professional compliance.
Establish and maintain open communication channels for ongoing feedback.
Support and contribute to team development needs and obligations.
MANDATORY TRAINING
All relevant employees of Yerrabi Yurwang are required to complete Mandatory Training and program specific training upon commencement of employment and complete refresher training as required.
SELECTION CRITERIA

Diploma or higher qualification in Community Services, Social Work, or related field.
Demonstrated experience in management or outreach support in DFSV, homelessness, or related sectors.
Robust understanding of trauma-informed and family-centred practice.
Experience working with women and children from Aboriginal and Torres Strait Islander families and communities.
Ability to work independently across multiple sites and in partnership with external agencies.
Excellent communication, advocacy, and organisational skills.
Current Working with Vulnerable People (WWVP) registration or equivalent.
Current driver's license and history, and access to a reliable vehicle.
Lived experience of DFSV or homelessness is valued where Yerrabi Yurwang and stakeholders can offer a range of appropriate supports to staff.
Experience with Family Journey.
