Queensland Health is seeking a Physiotherapist to join the Transition Care Program in Cairns. You will provide an effective and effective physiotherapy service to older people, and their carers and families, discharged from acute and sub-acute care settings into the Transition Care Program.
Working within a multidisciplinary team, you will conduct comprehensive assessments, deliver evidence-based programs, liaise with stakeholders, and contribute to program development and quality activities.
