NewDirection Care is seeking an experienced Registered Nurse to join our Bellmere residential care community as a Clinical Care Manager.
Working closely with the General Manager, you will provide clinical leadership and operational guidance to our Registered Nurses and House Companions. You will help ensure residents receive safe, individualised and high-quality care while supporting our vision of enabling each person to enjoy "as normal a life as possible."
This is a hands-on leadership position for someone who can balance clinical governance, team development and quality improvement while remaining genuinely connected to residents, families and everyday care delivery.
About the role
As Clinical Care Manager, you will help coordinate and oversee the care provided across the community. You will guide team members, maintain strong clinical standards and support compliance with the Aged Care Quality Standards, NDIS Quality Standards and relevant professional and legislative requirements.
Your responsibilities will include:
Providing clinical leadership, direction and support to Registered Nurses and House Companions
Working with the General Manager to coordinate safe and consistent care
Supporting resident assessments, care planning, case conferencing and care-plan reviews
Ensuring clinical documentation remains accurate,
current and compliant
Guiding the team in relation to AN-ACC documentation and processes
Overseeing medication-management practices, including Medication Advisory Committee meetings and RMMR outcomes
Supporting the management of complex clinical needs, including wound care, diabetes, behaviours, nutrition and hydration, pressure care and continence
Liaising with residents, families, medical practitioners, pharmacy and allied-health professionals
Supporting early and constructive resolution of clinical questions and concerns
Reviewing incidents, risks, quality indicators and clinical data
Participating in audits, accreditation activities and continuous-improvement initiatives
Supporting staffing, orientation, performance development and retention strategies
Coaching, mentoring and educating team members to strengthen clinical capability
Supporting graduate nurses and less-experienced employees
Providing after-hours support to the Registered Nurse team when required
The Clinical Care Manager may also work in a Registered Nurse capacity and provide direct clinical and personal care. This ensures leadership remains connected to residents, team members and the practical delivery of care.
About you
You are an experienced and approachable clinical leader who builds confidence in others. You can maintain clear standards while creating a supportive environment where employees feel comfortable asking questions,
raising concerns and developing their skills.
You will bring:
A Bachelor of Nursing

A minimum of three years' experience in a relevant supervisory or management position, preferably within aged care or community care

A robust commitment to quality improvement and resident-centred care
Sound clinical judgement and experience supporting residents with complex care needs
Strong written communication, documentation and reporting skills
Confidence using electronic clinical and care-management systems
Current NDIS Worker Screening clearance
COVID-19 and annual influenza vaccinations in accordance with applicable requirements
