Western Health in Melbourne is seeking a Diabetes Clinical Nurse Consultant (CNC) to educate inpatients, outpatients and community patients living with diabetes. The role may include ward-based education and in-services for staff.

It is based at Sunshine Hospital and involves adaptable delivery across care settings. The CNC will support safe discharge planning, assist clinics with glucose management and deliver home visits.
