Uniting NSW.ACT is hiring a permanent full time Care Partner to coordinate person-centred home and community care for a portfolio of clients.
The role focuses on delivering compatible care plans, building relationships and connecting clients with the right services to keep them safely at home.
You will travel between Uniting community sites and client locations, maintaining accurate records and working with internal teams to ensure high-quality care.
