Uniting AgeWell in Melbourne is seeking a Care Advisor (Case Manager) to lead a rapid-paced, client-centered role within the WestMetro Melbourne Home Care Program. You will manage your own caseload to promote independence and help clients live confidently at home.

You’ll work from Braybrook with a supportive team focused on positive outcomes and community impact. This full-time role runs Monday–Friday, 9am–5pm, balancing teamwork with meaningful work.
