Wintringham’s Home Support team in Shepparton is seeking an experienced Care Partner to deliver person-centred care management and service coordination that supports older people to maintain independence, wellbeing and quality of life in the community.

You will develop care plans, connect participants with the right services, build trusted relationships with participants, families and providers, monitor wellbeing, manage budgets and records, and escalate complex matters when needed.
