The Role
The McCusker Nurse offers expert advice, support, and referrals to carers of people living with dementia in metropolitan Perth, regardless of their aged care provider.
They act as a key resource, providing guidance and fostering connections with dementia specific agencies to provide support.
This position is being offered on a Full time basis working 76 hours per fortnight from 8:30am to 4:30pm based at our Corporate Office in Subiaco
Key responsibilities:
Provide dementia-specific person centred, information, education, and/or counselling, including telephone, written and face-to-face communication.
Provide expert advice, and resources to support the carers of people living with dementia.
Establish and maintain positive and professional relationships with people living with dementia, their carers, and the broader community.
Provide advocacy, and support for people living with dementia and their caregivers.
Ensure a timely and seamless referral process for clients to external and internal service providers
Implement a regular survey which will collect, and report on outcome measure data from the service provided
Maintain client record management and ensure reporting is in accordance with Amana Living's policies and procedures.
Where possible, provide education about dementia and McCusker Nurse services to Aged Care Service Providers, Aged Care Assessment teams, and other community groups.
Present at professional conferences and workshops as requested, to promote and raise awareness of Amana Living's role in dementia services.
About you




The ideal candidate will be an experienced registered nurse with extensive experience in the aged care sector, particularly supporting carers of people with dementia.
They will possess robust knowledge of dementia and related services, along with excellent clinical reasoning and decision-making skills.
Reporting to the McCusker Manager the candidate will be part of a team where they will have the skills to work autonomously, conduct telephone interviews, assess client needs effectively, and adapt to varying communication abilities.
Proficiency in computer literacy, including Microsoft Office and other database programs, is essential.
Essential Criteria:
Evidence of Current Flu vaccinations per Amana Living policies
Ability to obtain a National Police Clearance (within 6-months validity)
Successful completion of pre-employment form and reference checks
Degree in Nursing
Current APHRA Nursing Registration without restrictions
Western Australian C Class Driver's License
Experience working in Aged Care
Experience with Dementia care
