About the role

We are currently seeking a Full Time Care Partner.

The Care

Partner will be responsible for the onboarding and ongoing management of clients receiving Support at Home funding.

Partner provides high-quality, person-centred care management through assessment, care planning, service coordination, monitoring and review, promoting client wellbeing, independence, wellness and re-ablement in accordance with the Strengthened Aged Care Quality Standards. Care Partners at Corumbene work collaborative with older people and their family and friends to live the life that they choose.

Partners help support people to live with dignity and access the services they need, in a manner the suits individual and cultural preferences.

Key responsibilities

- Manage the onboarding and ongoing care management of Support at Home clients, ensuring continuity and coordination of services.
- Undertake comprehensive initial and ongoing assessments, care planning, reviews,



and referrals to meet changing client needs.
- Promote client choice, independence, wellness, re-ablement, and informed decision-making in all aspects of care and service delivery.
- Monitor client wellbeing, service delivery, risks, and changes in health status, responding promptly to concerns and clinical deterioration.
- Coordinate services with staff, families, health and community service providers and other stakeholders.
- Maintain accurate, comprehensive, and up-to-date client records, care plans, reports, referrals, and documentation in accordance with legislative and professional requirements.
- Develop and maintain qualified and therapeutic relationships with clients, families, carers, representatives, and external service providers.
- Ensure care and service delivery complies with the Strengthened Aged Care Quality Standards.
- Identify and contribute to quality improvement, professional development, education, training, and continuous improvement activities.

Essential Skills and Qualifications

- Demonstrated experience in care management, care coordination, assessment and care planning, preferably within aged care, support at home or community services.
- Relevant qualification in Community Services, Care Management, Aged Care or a related field, or equivalent combination of qualifications and relevant experience.




- Strong understanding of person-centred care, wellness, re-ablement, independence and choice.
- Excellent time management, organisational, prioritisation and problem-solving skills..
- Competence in electronic client management, documentation and care planning systems.
- Preparedness to work within the Vision, Purpose and Values of Corumbene.
- Robust interpersonal and communication skills, with the ability to engage effectively with consumers, families, carers, staff and external stakeholders.
- Demonstrated ability to assess and manage risk, respond to changing client needs and coordinate appropriate services and supports.
- Commitment to personal and professional development.
- Current National Police Certificate that satisfies the requirements of the Aged Care Act.
- Current Tasmanian Driver Licence.
- Working with vulnerable people card or ability to obtain.

Desirable

- Diploma of Community Services (Case Management) or equivalent qualification/experience.
- Understanding of the National Aged Care Quality Standards.
- Experience working within the Support at Home program.
- Experience coordinating multidisciplinary services and working with My Aged Care and aged care assessment services.
