The Opportunity:

Join our Health team as a FIFO-Chronic Conditions Nurse and play a key role in improving chronic disease outcomes for Yarnangu across the Ngaanyatjarra Lands. This is a unique opportunity to provide high-level clinical support for people living with complex and chronic conditions, while working alongside local clinic teams, GPs, Aboriginal Health Practitioners and visiting specialists across remote communities.

You’ll be a key clinical resource across the Lands, supporting chronic conditions management, recalls, health education and integrated care, while helping strengthen clinical systems and drive continuous improvement. If you’re an experienced nurse looking for meaningful work, professional autonomy and the opportunity to make a genuine difference in remote Aboriginal health, this role offers a rewarding FIFO opportunity in some of WA’s most remote communities.

About You:

We’re looking for an experienced nurse lwho is passionate about improving chronic health outcomes for Aboriginal people and is confident working in a remote primary health care environment.

Essential:

- AHPRA registration in the relevant health discipline.
- Experience in Aboriginal Health or remote Primary Health Care, with knowledge of chronic disease management and Preventable Chronic Disease frameworks.
- Strong understanding of Aboriginal health, Primary Health Care principles and culturally safe practice.
- Strong communication and interpersonal skills, with the ability to work effectively across multidisciplinary and culturally diverse teams.
- Demonstrated ability to manage and support change in a complex remote practice environment.
- Commitment to quality improvement, evidence-based practice, education and ongoing professional development.
- Proficiency with patient information systems, clinical data and other information technology.
- Current manual driver's licence ability to obtain and maintain a National Police Clearance and WA Working with Children Check.

Desirable:





- Postgraduate qualifications in chronic disease management, diabetes education, rural and remote health or a related clinical discipline.
- Experience delivering chronic disease self-management education and support.

Home to approximately 2000 Yarnangu (Ngaanyatjarra people) across 11 unique communities, our council is dedicated to preserving and celebrating our rich heritage spanning over 10,000 years and 250,000 square kilometres.

Established in 1981, the Ngaanyatjarra Council Group (NCG) represents the interests of the Ngaanyatjarra People across the 11 communities in the Central Desert region of Western Australia. Employing in excess of 450 people, NCG provides a wide range of services, including health, utilities, construction, employment, community development, shops, roadhouses, art centres and land services. It is one of the largest and most successful Indigenous organisations in the country.
