Your opportunity:
- To provide expert clinical and patient-centred care within Integrated Care Rehabilitation Services by supporting the facilitation and provision of quality and cost effective nursing services which are consistent with contemporary standards and evidence-informed practice.
- The Transition Care Program is an exciting jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time-limited and goal oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub-acute hospital admission and live within the Wide Bay Health Service region.

Our offer:
You will receive a competitive remuneration package value up to $139,931 p.a., comprising salary between $114,532 - $122,643 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)) (Two Permanent Full Time Positions, 76 hours p.f.)
