About Us

All About Your Support Pty Ltd is a disability support provider committed to delivering respectful, person-centred and high-quality support to people living with disability. We aim to empower participants to increase their independence, participate in their community and achieve their individual goals.

We are currently seeking compassionate, reliable and motivated Community Workers to join our growing team.

About the Role

As a Community Worker, you will provide individualised support to NDIS participants in their homes and within the community. You will work closely with participants to promote independence, choice, dignity and meaningful community participation.

Your responsibilities may include

- Supporting participants with community access and social activities
- Assisting participants to attend appointments, shopping, recreational activities and other community engagements
- Providing assistance with daily living activities and personal care where required
- Supporting participants to develop independent living and social skills
- Assisting with meal preparation and light household duties
- Providing transport and accompanying participants to appointments and activities
- Supporting participants to work towards their NDIS goals
- Providing emotional and social support while maintaining professional boundaries
- Following individual support plans, behaviour support plans and organisational procedures
- Completing accurate progress notes, incident reports and other required documentation
- Identifying and reporting changes in a participant's wellbeing,



behaviour or support needs
- Maintaining participant privacy, dignity, confidentiality and choice
- Following workplace health and safety, infection control and risk-management procedures
- Working collaboratively with participants, families, support coordinators and other members of the support team

About You We are looking for someone who is caring, patient, respectful and genuinely passionate about supporting people with disability.

The successful candidate should demonstrate:

- Excellent communication and interpersonal skills
- A respectful and person-centred approach
- Reliability, punctuality and professionalism
- Ability to work independently and as part of a team
- Understanding of professional boundaries and confidentiality
- Ability to follow support plans and organisational policies
- Good written communication skills for completing progress notes and reports
- Flexibility to work a range of shifts, including weekdays, evenings and weekends where required

Requirements Applicants should have, or be willing to obtain where applicable:
- Certificate III or IV in Individual Support, Disability, Community Services or a related qualification (preferred)




- NDIS Worker Screening Check
- National Police Check
- Working with Children Check, where required
- Current First Aid and CPR certification
- NDIS Worker Orientation Module
- Current Australian driver's licence
- Reliable and insured vehicle, where required for participant transport
- Australian working rights

Previous experience in disability support, community services, aged care, mental health or a similar role will be highly regarded.

Pay: $75,000.00 – $85,000.00 per year
