About the Role

We are seeking an experienced Community Worker to join our team on a full-time basis.

This role focuses on understanding participants’ individual support needs, developing appropriate support strategies, connecting them with community resources and services, and helping them build the skills and connections needed to achieve their goals.

You will work collaboratively with participants, families, health professionals, community organisations and other service providers to ensure supports remain responsive to each participant’s needs and circumstances.

Key Responsibilities

- Identify participants’ individual, social and practical support needs and contribute to the development and review of individualised support plans.
- Provide information and guidance on suitable community services, programs and support options.
- Assist participants to navigate and access relevant health, education, employment, social and community services.
- Identify appropriate service providers and community resources, coordinate referrals and follow up on outcomes.
- Advocate for participants’ rights, choices and preferences when engaging with service systems and community organisations.




- Plan and facilitate community-based activities and support programs that promote wellbeing, social inclusion and skill development.
- Support participants to develop life skills, confidence and community connections.
- Liaise with families, support coordinators, health professionals, government agencies and other service providers to support coordinated service delivery.
- Monitor participants’ progress and changing circumstances, review the effectiveness of existing supports and recommend adjustments or additional referrals where appropriate.
- Maintain accurate and confidential client records, case notes, referral information and reports.
- Work in accordance with NDIS requirements, relevant standards and organisational policies and procedures.

Requirements

- A Diploma in Community Services, Disability Services, or a related field.
- At least one year of relevant experience in a Community Worker role.
- Sound knowledge of the NDIS, community support systems and referral pathways.
- Strong communication, stakeholder engagement and organisational skills.
- The ability to identify support needs and develop appropriate strategies and referrals.
- Strong documentation and case-note management skills.
- The ability to work independently while collaborating effectively with participants, families and service providers.
- A genuine commitment to person-centred practice, dignity, respect and inclusion.
