Alana Care Support Services pty limited Key Responsibilities The successful applicant will be responsible for: Assessing the needs of clients and identifying appropriate community, health, welfare, housing and social support services. Supporting clients to identify their individual goals and develop strategies to improve their independence and community participation. Providing information, guidance and referrals to appropriate community services and support organisations.

Liaising with community organisations, welfare agencies, government bodies, health professionals and other service providers on behalf of clients. Assisting clients to access suitable community programs, educational activities, employment services and recreational opportunities. Developing and maintaining relationships with local community organisations and service providers.

Coordinating community-based activities and programs that promote social inclusion and participation. Monitoring clients' progress and reviewing their changing support and community service needs. Advocating for clients and assisting them to overcome barriers to accessing community resources and services.

Maintaining accurate client records, case notes and reports in accordance with organisational and regulatory requirements. Contributing to the development and improvement of community programs and services. Promoting awareness of available community resources and support services.

Skills and Experience The successful candidate will have: Relevant qualifications in Community Services, Social Services, Welfare, or a related field.



Relevant experience in community services, disability services, welfare or a similar workplace. Strong communication and interpersonal skills.

Ability to assess client needs and coordinate appropriate services and referrals. Ability to communicate effectively with clients, families, community organisations and other service providers. Good organisational, documentation and case management skills.

A person-centred and culturally sensitive approach. Ability to work independently and as part of a multidisciplinary team.
