About the Role – Disability Support Worker / NDIS Support Worker – Mandarin Speaking Preferred
Caring Your Home is seeking reliable and caring Disability Support Workers to provide direct support to NDIS participants.
In this role, you will assist participants with daily living activities, personal care, community access, household tasks, and skill development. The aim of the role is to support participants to maintain their independence, dignity, safety, wellbeing, and quality of life.
You will work with participants in their homes and in the community, following their individual support plans and reporting any concerns or changes in their needs.
Key Responsibilities
- Assist participants with daily living activities, including showering, dressing, grooming, toileting, mobility, transfers, and other personal care needs.
- Support participants with meal preparation, feeding assistance, hydration, and medication prompting where required and within your scope of work.
- Assist with domestic tasks, including cleaning, laundry, changing bed linen, shopping, and general household support.
- Support participants to access the community, attend appointments, participate in social activities, and engage in recreational programs.
- Assist participants to develop daily living skills, such as personal hygiene, cooking, budgeting, communication, and community participation.
- Encourage participants to work towards their personal goals, including independence, social participation, education, or employment-related goals.
- Follow individual support plans, care plans, behaviour support strategies, and company policies and procedures.
- Maintain accurate service notes, progress notes, incident reports, and other required documentation.
- Report any concerns, incidents, changes in participant condition, risks, or safeguarding concerns to the office or management team.
- Work respectfully with participants, families, carers, support coordinators, healthcare professionals, and other service providers.
- Attend training, supervision, and team meetings as required.
About You
We are looking for someone who is reliable, respectful, patient, and genuinely interested in supporting people with disability.
The successful applicant will have:
- Certificate III or Certificate IV in Disability Support, Individual Support, Aged Care, Community Services, or a related field, preferred.
- Previous experience in disability support, aged care, personal care, community care, or healthcare support is highly desirable.
- Understanding of NDIS participant rights, choice and control, privacy, dignity, and person-centred support.
- Good communication and interpersonal skills.
- Ability to follow care plans, instructions, and workplace procedures.
- Ability to work independently and report issues appropriately.
- A caring, respectful, and professional attitude.
- Flexibility to work across different shifts, including weekdays, weekends, evenings, or public holidays where required.
Essential / Preferred Requirements
- Valid NDIS Worker Screening Check, or willingness to obtain.
- National Police Check, or willingness to obtain.
- First Aid and CPR certificate, or willingness to obtain.
- Working With Children Check, if supporting child participants.
- Valid Australian driver licence and access to a reliable vehicle is highly desirable.
- Mandarin and/or Cantonese speaking ability is highly regarded.
- Previous experience supporting participants with physical disability, psychosocial disability, intellectual disability, autism, complex care needs, or mobility support needs will be highly regarded.

- 根据参与者需要及您的工作范围,协助准备餐食、进食支持、饮水提醒及用药提醒。
- 协助完成日常家务,包括清洁、洗衣、更换床单、购物及其他家庭支持工作。
- 支持参与者外出进入社区、参加预约、参与社交活动及合适的娱乐项目。
- 协助参与者发展日常生活技能,例如个人卫生、烹饪、预算管理、沟通能力及社区参与能力。
- 鼓励参与者实现个人目标,包括提升独立性、社交参与、学习或与就业相关的目标。
- 按照个人支持计划、护理计划、行为支持策略及公司政策和流程提供服务。
- 准确完成服务记录、进展记录、事件报告及其他所需文件。
- 及时向办公室或管理团队汇报任何问题、事件、参与者身体或行为变化、风险或安全保护相关事项。
- 与参与者、家属、照护者、Support Coordinator、医疗专业人员及其他服务提供方保持尊重和专业的沟通。
- 按要求参加培训、督导及团队会议。
任职要求
- 持有 Certificate III 或 Certificate IV in Disability Support、Individual Support、Aged Care、Community Services 或相关领域证书者优先。
- 有残障支持、养老护理、个人护理、社区护理或医疗支持相关经验者优先。
- 理解 NDIS 参与者的权利、选择与控制、隐私、尊严、以人为本的支持原则。
- 具备良好的沟通能力和人际交往能力。
- 能够遵守护理计划、工作指引及公司流程。
- 能够独立工作,并在需要时及时汇报问题。
- 具备关怀、尊重和专业的工作态度。
- 能够根据服务需求灵活工作,包括工作日、周末、晚班或公共假期。
必备或优先条件
- 持有有效的 NDIS Worker Screening Check,或愿意申请。
- 持有 National Police Check,或愿意申请。
- 持有 First Aid and CPR 证书,或愿意申请。
- 如服务儿童参与者,需持有 Working With Children Check。
- 持有澳洲有效驾照并有可靠车辆者优先。
- 会普通话和/或粤语者优先。
