HammondCare Nowra is seeking a Care Partner to deliver and develop tailored Care Plans for each client, overseeing ongoing assessments and ensuring services meet funding and budget expectations. The role is full-time, Monday to Friday at the HammondCare at Home Nowra Office, supporting growth.

You will manage care plans with stakeholders, support client KPIs, maintain up-to-date plans, and provide qualified day-to-day care delivery.
