The Support Worker provides person-centred care and support to older people and or people with disability living in the community.
The role focuses on assisting clients to maintain their dignity, wellbeing, safety, and independence while remaining in their own homes and engaged in their community.Key responsibilitiesAssist clients with personal care tasks, including showering, dressing, grooming, toileting, and continence supportSupport mobility, transfers, and safe movement within the homeEncourage clients to maintain independence and choice in daily activitiesProvide assistance with light household tasks, including cleaning, laundry and bed makingSupport meal preparation in line with dietary requirements and preferencesAssist with grocery shopping and essential errands as outlined in the care planProvide companionship and emotional supportEncourage and support participation in social, recreational, and community activitiesObserve, monitor,



and report changes in a client's physical, emotional, or cognitive conditionAccurately document care provided, observations, and client progressAbout youCertificate III in Individual Support (Ageing, Home & Community Care) or equivalentCurrent First Aid and CPR (or willingness to obtain)Current Police Check and relevant aged care/ NDIS screening checksValid driver's licence and access to a reliable vehicle (if required)Solid interpersonal and communication skillsEmpathy, patience, and respect for older peopleAbility to deliver person-centred, strengths-based careAbility to work independently and as part of a teamExperience supporting older people in a community or care setting (preferred)Experience working in home-based or non-residential aged care environments (desirable)
