ForHealth Group is seeking a Registered Nurse or Enrolled Nurse to join the Kedron Medical Centre on a part-time basis. You will lead Chronic Disease Management initiatives, coordinating with GPs and patients to improve health outcomes through preventative care and ongoing support.
In this role you will perform clinical services, coordinate GPMPs/TCAs, conduct health assessments and contribute to a team-oriented multidisciplinary team dedicated to high-quality primary care.
