The Transition Care Program are seeking a Physiotherapist to join them. In this role you will provide an effective and efficient physiotherapy service to older people, and their carers and families, who have been discharged from acute and sub-acute care settings and accepted into the Transition Care Program. This includes working collaboratively within a multidisciplinary team to case manage the wider needs of allocated clients.

About your role
- Provide competent comprehensive physiotherapy assessment, intervention and case management of clients referred to the Transition Care Program including liaison with key stakeholders.
- Demonstrate communication and teamwork skills, by ensuring effective liaison with Transition Care Program clients, their families, other health professionals, supervisors, students and researchers, as required.
- Deliver evidenced based physiotherapy programs to a frail elderly client case load.
- Participate in the development of the Transition Care Program and Physiotherapy quality activities, clinical practices, procedures and protocols.
