Eastern Health in Melbourne's east is seeking a Clinician-Case Manager to join Chandler House Continuing Care. You will provide highly responsive case management for adults with severe mental illness, coordinating care with consumers, families and a multidisciplinary team to support recovery and independence.
You will conduct biopsychosocial, mental state and risk assessments, develop care plans, and work across acute, rehabilitation and community services to connect clients with their supports.
