About the role
Hospital in the Home (HITH):
Provides acute hospital-level care to patients in their own homes as a safe alternative to inpatient admission.
Practice in line with NSW Health guidelines, local policies, and protocols.
Deliver evidence-based nursing care to people of all ages and cultural backgrounds, including individuals, families, and communities.
Assess, plan, implement, and evaluate care collaboratively to achieve health goals and optimal outcomes.
Lead and coordinate nursing and healthcare across settings, including referrals and collaboration with other professionals and services.
Practice independently and collaboratively, maintaining accountability and appropriately delegate care based on staff education, training, and context.
About you
Currently registered as a Registered Nurse (division 1) with the Australian Health Professionals Regulation Agency (AHPRA).
Current NSW unrestricted driver's license (class c or above) in order to attend client's home with provided car.
Experience working within community healthcare settings is highly desired.
Demonstrated ability to work within an interdisciplinary team using effective communication and interpersonal skills.
Demonstrated effective time management skills and ability to prioritise competing workloads within a complex setting.
