About the Role
We are seeking an experienced, proactive, and professionally confident NDIS Program - Care Coordinator to join our team in Pennington. In this critical role, you will take ownership of participant care coordination, manage complex practice and clinical risks, and provide frontline guidance to our Disability Support Workers. Working closely with the Program Manager and Administration team, you will ensure our service delivery is safe, high-quality, and tailored to each participant’s goals.
This hands-on position suits a skilled case manager, nurse, or allied health professional who excels at problem-solving, building trust with multidisciplinary teams, and translating complex assessments into actionable support plans.
Key Responsibilities
- Participant Intake & Care Planning: Lead participant intake, conduct holistic risk and care assessments, and develop, implement, and regularly review comprehensive support plans.
- Clinical & Practice Supervision:
Monitor medication management, behaviour support plans, restrictive practices, and emerging healthcare needs, making timely referrals to external specialists and allied health professionals.
- Workforce Mentoring & Competency: Provide practice-level coaching, guidance, and formal competency assessments for Disability Support Workers to build frontline workforce capability.
- Quality & Audit Compliance: Conduct routine audits of participant documentation and service records to uphold NDIS Practice Standards and drive continuous quality improvement.
- Multidisciplinary Coordination: Work collaboratively with participants, families, Support Coordinators, and healthcare providers to ensure cohesive service delivery.
- On-Call Support: Participate in a rotating after-hours on-call roster to support urgent participant and operational matters.
Requirements
- Qualifications: Tertiary qualification in Nursing, Social Work, Allied Health, or a related community services discipline (AHPRA registration desirable).
- Experience: Proven experience in care coordination, case management, or clinical supervision within disability, aged care, or health sectors.
- Sector Knowledge: Thorough understanding of NDIS Practice Standards, participant safeguarding, risk management, and complex support needs.
- Clearances & Checks: Current Australian Driver’s Licence, NDIS Worker Screening Check, Working with Children Check (WWCC), National Police Check, and First Aid/CPR certifications.
- Core Capabilities: Outstanding written and verbal communication skills, high attention to detail, strong problem-solving skills, and full Australian working rights.
Pay: From $91,065.00 per year
