About the role

Alphacare Services is seeking an experienced and dedicated NDIS Support Coordinator to join our team in Melbourne. This full-time role will be responsible for providing high-quality support coordination services to clients with disability, helping them to access and utilise their NDIS funding effectively.

Salary Range: $83,000 – $90,000 per year

What you'll be doing

- Develop and implement comprehensive support plans to meet the individual needs of NDIS participants
- Coordinate access to a range of support services, including allied health, accommodation, employment, and community engagement
- Advocate on behalf of clients to ensure they receive the appropriate level of funding and support
- Regularly monitor and review client progress, making adjustments to support plans as needed
- Maintain detailed records and provide regular reports to clients, their families, and the NDIS
- Collaborate with a multidisciplinary team of health and community professionals
- Contribute to the continuous improvement of our NDIS support services

What we're looking for

- Minimum 2 years' experience in NDIS support coordination or a related field
- Thorough understanding of the NDIS framework, operational guidelines and eligibility criteria
- Excellent communication and interpersonal skills, with the ability to build rapport with a diverse range of clients
- Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for clients
- Demonstrated ability to work independently and as part of a team
- Diploma of Community Services or relevant tertiary qualification in disability services, social work, or a related field




- Current driver's licence and access to a reliable vehicle

We provide Core Supports and Support Coordination, Short Term Accommodation (STA), and supported independent living (SIL) for eligible NDIS participants.
