You will work across a multidisciplinary environment, building strong partnerships with clinicians and services within Western Sydney Local Health District and beyond.
Work within a major tertiary hospital and specialist cancer service
Support patients with complex Head & Neck and Central Nervous System cancers
Collaborate across multidisciplinary teams and public and private healthcare networks
Contribute to specialist education, evidence-based practice and professional development
Influence research,
quality improvement and the future delivery of cancer care
What you'll be doing
As Cancer Care Coordinator, you will provide advanced clinical consultation and complex care coordination, helping patients, families and carers navigate their cancer journey. You’ll combine expert nursing practice with clinical leadership, education, research and service development.
Provide expert clinical advice, consultation, assessment, care coordination and education
Lead the planning, implementation and evaluation of specialist nursing care and clinical practice
Develop and deliver education for patients, carers, nurses and multidisciplinary healthcare professionals
Initiate and contribute to research, quality improvement and best-practice initiatives
Analyse current models of care and influence service planning, policy, guidelines and strategic improvements
