ForHealth Group is seeking a Registered Nurse (RN) or Enrolled Nurse (EN) to join our Chronic Disease Management team in Rockhampton.
In this role you will work with GPs and allied health professionals to support patients with chronic conditions through education, care coordination, and ongoing support.
You will conduct clinical services such as health coaching, care planning, and health assessments, with a focus on preventative care and improving long-term health outcomes.
