Job Description
TLC Healthcare is seeking a General Practice Nurse to join our Altona North team on a full time basis.
You will work with GPs, the Practice Manager, and a multidisciplinary team to deliver high-quality patient-centred care and support improved health outcomes for the community.
Key responsibilities include nursing care, patient assessments, care planning, and supporting health promotion, chronic disease management, and preventive care.
