ACH Group is seeking an experienced Registered Nurse to join as a Care Partner (Clinical).
You will provide clinical guidance, oversight for complex care, and support a multidisciplinary team to improve customer outcomes in the community and residential settings.
You will conduct assessments, develop care plans, and coordinate post-discharge and palliative care as part of a person-centred approach.
Travel within the community is required.
