St Vincent's Care at Home in Toowoomba is seeking a skilled Registered Nurse to deliver acute-equivalent nursing care in community settings. You will assess needs, develop tailored care plans and coordinate protected transitions of care for patients receiving care at home.
You will collaborate with MCAH Lead, VMOs and multidisciplinary teams to deliver coordinated, patient-centred care, reducing hospital admissions and improving outcomes while supporting flexible employment arrangements.
