Job Description
Western Health is recruiting for a part time Aged Care Assessor to perform comprehensive clinical and non-clinical assessments for Commonwealth-funded aged care services. You will develop person-centred support plans and coordinate referrals within a multidisciplinary team to support independence and wellbeing.
