Wise Healthcare is seeking reliable and caring Support Workers to assist an NDIS participant who has recently moved to Port Albert, VIC.

We are looking for workers who can provide consistent practical, social and community-based support and help the participant settle into her new community.

Available Shifts

Home Setup & Community Access – Male Support Worker Preferred

-Three days per week (4 hours per shift)

-Days to be discussed based on availability

-Support includes home organisation, unpacking, light gardening, community access and helping the participant establish herself locally.

Social & Capacity-Building Support – Mature-Aged Female Preferred

-Two Days per week (4 hours per shift)

-Days and times to be discussed based on availability

Support includes transport to Yarram, appointments, social activities, community access and building confidence and independence.

About You

We are looking for someone who is:
Reliable, patient and respectful
Experienced in disability or community support
Committed to supporting participant choice,



dignity and independence
Holds a current driver's licence and has access to a reliable, insured vehicle
Willing to obtain an NDIS Worker Screening Check, First Aid and CPR if required
Able to provide relevant references

Additional Prospects

We are also seeking:

-Cleaners for regular weekly cleaning
