Omnia Community Health is seeking an Enhanced Care Coordinator to support young people aged 12–25 with complex mental health needs in Sunbury and the Macedon Ranges. The role focuses on assessment, intervention, and referrals within a multidisciplinary counselling team.

You will manage a diverse caseload, collaborate with other health specialists, and provide services across multiple local settings to improve client outcomes.
