About the role The Coordinator of Individualised Services will manage a range of support services to assist people with disabilities to live the "valuable life".
Key responsibilities
Act in a person centred manner at all times
Ensure through coordination and leadership that GIFSA is delivering high quality supports and services
Assume responsibility for the people we support's roster and staffing matches
Regularly review the services and supports provided to the people we support, ensuring their needs are being met
Utilise effective work practices to encourage choice and control for the people we support
Ensure individual plans are person centred and reviewed as per deadlines
Build and maintain relationships through quarterly contact with the people we support and all relevant stakeholders
Ensure individual and group risk assessments are complete and documented where required
Build and maintain effective teamwork between support staff
Provide direction and mentoring to support staff, ensuring they are provided with feedback, coaching, training and support to improve performance
About you
Relevant experience in the Human Services industry
Ability to manage and lead a team and team leaders
Well-developed advocacy skills
Ability to build and nurture relationships at a personal, organisational and community level
Effectively manage and implement medically developed care plan requirements as directed by qualified specialists and specialists
Ability to set goals, manage a variety of tasks and achieve outcomes
Demonstrated computer skills including a working knowledge of databases, Microsoft Excel and Microsoft Word
Well-developed problem solving skills
Well-developed communication and interpersonal skills and the demonstrated capacity to work effectively within a team
Current First Aid Certificate, Current Working with Children Check, NDIS Workers Screening Check, and Current WA "C" class drivers licence
