ForHealth Group’s Ballarat Medical Centre is seeking a Registered or Enrolled Nurse to support GPs through chronic condition management and preventative care. You’ll join a supportive team focused on quality patient outcomes in Ballarat.

You will conduct health assessments, educate patients, coordinate referrals, and help grow CCM services while maintaining solid communication with the wider practice team. Part time or full time hours available.
