The Role: managing a community-based caseload, primarily supporting Aged Care clients (small proportion of NDIS participants)

Your responsibilities will include:
Functional assessments within clients' homes
Assistive technology prescription and equipment recommendations
Minor and major home modification assessments
Developing intervention plans to maximise independence and safety at home
Reviewing client progress and completing high-quality clinical reports
Working collaboratively within a multidisciplinary team

You'll need:
Current AHPRA registration as an Occupational Therapist
Current driver's licence and access to a vehicle
Robust communication and time management skills
A passion for delivering person-centred care and achieving outstanding outcomes for clients
