Uniting NSW & ACT is seeking a permanent full time Care Partner to coordinate person-centred home and community care for a portfolio of clients, helping them live safely at home.

You will build trusted relationships, develop and review care plans within funding, and act as the central contact for clients, families and providers.

Travel between Uniting community sites and client locations as required, and contribute to quality improvement and record-keeping using client management systems.
