ForHealth Group is seeking a Registered or Enrolled Nurse to join Bankstown Medical Centre, focusing on chronic condition management (CCM) and preventative healthcare.

You will support GPs and patients through CCM, health assessments, and health behaviour coaching within a collaborative, patient-centred team. Adaptable part time or full time hours are available, with opportunities to grow your career through training.

AHPRA registration is required, and previous CCM experience is preferred.
