Aveo is seeking a Care Partner (Clinical) to support residents in our Aveo retirement communities with in-home care. You will assess needs, develop care plans, and coordinate services to help residents live independently for longer—spending your day in residents' homes with a manageable caseload of around 45.
You'll work Monday to Friday with an Aveo vehicle, fuel card, laptop, and phone provided, along with comprehensive onboarding and ongoing support from the Care at Home team.
