Australian Unity is seeking a Care Partner to support customers on their care journey, coordinating services and developing person-centred care plans. This permanent full time role is based at our Taree branch, with a hybrid work setup including client visits, branch days and WFH.

You will manage a customer portfolio, conduct assessments, coordinate multidisciplinary teams and ensure positive outcomes through ongoing reviews and proactive planning.
