Job Description
Catholic Healthcare in Western Sydney seeks a Clinical Aged Care Assessor to join the ACAS team. The role involves conducting assessments to identify complex needs and referring clients to appropriate aged care services under the Aged Care ACT.

You will work with clients, families and colleagues to meet KPIs and maintain service standards. The position requires a health-related qualification and current AHPRA registration, plus a valid Australian driver's licence and travel to local sites.
