BlueCare is seeking a Care Partner to deliver high-quality, person-centred care in community settings across Recent Farm, Hamilton, Kedron, and surrounding areas.
You will work with clinical teams to plan, coordinate, and review care services for aged care clients.
Role requires a Certificate IV in Health or Community Service, a current AU driver's licence, and a roadworthy vehicle.
You'll travel regularly, liaise with care teams, and monitor client outcomes while maintaining autonomy and robust
