The role involves conducting person-centred assessments and coordinating referrals to Commonwealth-funded aged care services.
As a Registered Nurse or Allied Health skilled (OT/Physio/ST/SW), you will work within a multidisciplinary team, ensure accurate documentation, and contribute to high-quality, client-centred care while pursuing ongoing career development.
