LiveHire Limited in Victoria is seeking a Care Partner for the Support at Home (SAH) program on an ongoing part time basis (3 days per week). You will plan, manage and coordinate services aligned with aged care assessments and participant goals, collaborating with informal carers and family where appropriate.

The role requires experience in health and community settings and a Certificate III in health, nursing or a related field. Competitive remuneration and employee support are provided.
