Job Description
Mercy Health is recruiting for a Social Work / Care Coordination role within the Health Independence Program in the Wyndham catchment. Based in a agile office, you will work with Allied Health staff to support HIP clients in the community and outpatient settings.

The role involves home visits, comprehensive psychosocial assessments and coordinating care across services. Applicants should hold a Bachelor or Master of Social Work, be eligible for AASW membership, have a valid Victorian Driver's
