HammondCare invites applications for a Team Leader in Parramatta to guide the Hospital to Aged Care Dementia Support Program (HACDSP) within Dementia Support Australia (DSA). Lead a multi-site team, drive KPI-compliant service delivery, and oversee referrals, resources and stakeholder relationships.

You will provide skilled leadership, applying psycho-social approaches to dementia care and ensuring high-quality outcomes for people transitioning from hospital to home or residential care.
