Catholic Healthcare Limited is recruiting an Aboriginal or Torres Strait Islander Identified Aged Care Assessor for our ACAS team on the Mid North Coast. The position involves conducting assessments to identify needs and refer to appropriate home support services, with local travel across the region.

You will work with clients, families and multidisciplinary teams to promote a supportive culture, and ensure accurate assessments using the Integrated Assessment Tool (IAT) to determine client needs
