DPV Health is seeking a dedicated Care Finder to support seniors in Melbourne's Mitchell and Whittlesea areas. You will assist clients to understand and access aged care services, build partnerships with community providers, and guide clients through assessments to obtain needed supports.

The role requires a tertiary qualification in Human Services or related field, robust knowledge of aged care systems, and the ability to work autonomously while collaborating with teams.
